In Germany, mortality due to cardiovascular disease is disproportionately high compared to other countries. At the same time, it has one of the highest rates of coronary angiography in the world.

This paradoxical situation is comprehensively analyzed in the current review "Coronary heart disease - a German paradox" by Remppis et al.

The central problem: The guideline models used to date to estimate the pretest probability (PTP) of coronary heart disease (CHD) are imprecise and therefore unsuitable for efficiently controlling the diagnostic pathway.

The selection of suitable, non-invasive and invasive diagnostics essentially depends on the individually calculated PTP - its correct determination is the decisive step for efficient and patient-friendly treatment.

For the first time, Cardio Explorer® enables personalized PTP based on clinical data and 15 laboratory parameters. The publication shows that with a diagnostic accuracy (AUC) of 0.87, the Cardio Explorer® achieves performance on a par with imaging procedures - and enables significant cost savings .

The authors emphasize that AI-based tools such as Cardio Explorer® help to consistently implement the logic of the ESC 2024 guidelines: avoiding unnecessary diagnostic steps, stratifying patients in a more targeted manner and using valuable resources in the healthcare system more efficiently.

AI-based precision diagnostics is the key to sustainable, evidence-based and patient-centered cardiac medicine.
